Required documents to apply for a new passport

To apply for a new passport (from 8 months for the expiration date) the FOLLOWING DOCUMENTS are required:

  1. The full (in BLOCK LETTERS) personally signed and completed application form
    Please note: the form is not digitally available, you could request a form
  2. at the office, when submitting your request, or
  3. by letter, with inclusion of a prepaid envelope addressed to you.
  4. A clear (color) copy of the full passport (all pages and cover).
  5. A clear (color) copy of the residence document.
  6. Three (3) good, similar color passport photos (not older than 3 months);
  7. Face: natural view, frontal, both ears visible, neutral facial expression.
  8. Background: white/light grey, color difference background and clothing, no gradient.
  9. For males: no earrings.
  10. An original copy (not older than 6 months) indicating your current nationality and marital status from the Civil Registry (abbreviated as BPR) from your current residency, where you/the minor child is/was registered, with a signature of the municipality official, and an apostille stamp from the Court.
  11. For a minor born in Suriname: A copy of the birth certificate
  12. For a minor child born outside of Suriname: A recent original copy of the birth certificate with signature from the municipality official from the commune of residency and an apostille stamp from the Court
  13. For an adult who is born outside of Suriname: A recent, original copy of the birth certificate, with signature of the municipality official and a Court apostille
  14. A recent original copy of the marriage certificate with a signature from the municipality official, a stamp, and in case the marriage was outside of suriname: a Court stamp.
  15. A recent original copy of the divorce certificate with signature of the municipality official, a stamp, and in case the divorce took place outside a Surinamese court, an apostille from the Court.
  16. An original copy of the death certificate, with v/d v municipality official, signature stamp v/d municipality v Court, where an apostille you widow/widower.
  17. In case of the loss of a Passport;
    -the original minutes of the police, with the signature of the declarant and the verbalisant (police officer) and police stamp and a copy of your valid ID;
  18. In the absence of proof of your identity, a declaration will need to be submitted.
  19. In case of a damaged passport; the damaged passport and a private Declaration on the cause, with your signature.

Note: children of age 6 years or older sign their signature (writing their first name is allowed) in the appropriate box on the form.

PLEASE NOTE: For minors under the age of 18 years, who have not been previously married, the application form needs to be co-signed (at the bottom of the form "name father/mother/guardian (es)") by:

(i) both parents; in case of a legitimate or legitimized child, to produce:
-an original copy (not older than 6 months) from the BRP (indicating the current nationality and marital status) of both parents, with signature of the municipality official, municipality stamp and an apostille from the Court.

  • in case the parent(s) have Dutch nationality: a statement or extract from the municipality with date of obtaining Dutch nationality with the signature of the municipality official, and an apostille stamp of the Court.

-a clear copy of pages 2 and 3 of the valid passport or clear copy of photo ID from both parents.

(ii) the mother; in case of a natural child or a recognized child, to produce:
-an original copy (not older than 6 months) from the BRP (indicating the current nationality and marital status) of the mother, with signature of the municipality official, municipality stamp and an apostille from the Court.

-in case the mother has Dutch nationality; a statement or extract from the municipality with date of obtaining Dutch nationality with the signature of the municipality official, and an apostille stamp of the Court.

-a clear copy of pages 2 and 3 of the valid passport or a clear copy of photo ID of the mother.

(iii) guardian(es); in the case of custody of the child, one has to produce:
- a clear copy of the custody decision.
- production of a recent extract from the Gezagsregister v/d Court (not older than 1 month) with an Apostille.
-a clear copy of pages 2 and 3 of a valid passport or a copy of a photo ID from the guardian(es)

(iv) For a permission letter applying for a passport for the minor child.
The signature of the parent (s)/guardian (es) in the permission letter should be legalized;
-holders of a Dutch passport/identity proof for the legalization of their signature at the municipality in their hometown. Then the signature v/d to be legalized by means of an apostille local government official with the Court.

-holders of a Surinamese passport can visit the Consulate, on presentation of their valid Surinamese passport.

TRANSLATION
Documents of which the text is not written in Dutch or English, must be translated by a certified translator to Dutch or English. Then, the signature of the translator needs to be legalized by means of an Apostille.

APOSTILLE
Documents issued by foreign authorities, including the Netherlands, must be not older than 6 months, and needs to have the legalized signature by a local government official and an apostille at the Court or competent authority.

PASSPORT PICK-UP
As soon as the requested Passport is ready, you will receive written notification. You will need to pick up the passport in person.
